Health Coach

ThedaCare is seeking a Health Coach to facilitate health promotion programs, assist team members and spouses in identifying health risks, and support them in achieving personal wellness goals. This role involves providing individual and group coaching, developing effective health strategies, and promoting a culture of health and well-being within the organization and community.

Key Responsibilities

• Review health risk assessments to help participants understand lifestyle-related health risks and assess readiness for change.
• Utilize motivational skills to set health-related goals and develop strategies to achieve them.
• Provide health coaching in individual and group settings.
• Document goals, clinical data, and behavioral outcomes in the electronic medical record.
• Collaborate with leaders, wellness teams, departments, and external clients to promote and support wellness initiatives.
• Plan and assist with health-related demonstrations, programs, displays, and materials to support individuals in their wellness journey.
• Implement, promote, and lead health, wellness, and chronic disease programs emphasizing healthy lifestyles, such as weight management, nutrition, smoking/nicotine cessation, stress reduction, and fitness evaluations.

Qualifications

• Bachelor's degree in a health-related field.
• Experience in health coaching or wellness program facilitation.
• Strong interpersonal and motivational skills.
• Ability to work collaboratively with diverse teams and individuals.
• Proficiency in electronic medical record documentation.
