Veteran Outreach Wellness Coach (Social Worker I BSW)
The Medical College of Wisconsin seeks a Veteran Outreach Wellness Coach to join the Captain John D. Mason Veteran Peer Outreach program. In this role, you will coordinate patient care activities, collaborating with a multidisciplinary team to support veterans' mental health and well-being.
Key Responsibilities
- Monitor patient care from initial community contact through referrals to outpatient, inpatient, or emergency services.
- Collaborate daily with peer specialists to ensure quality care.
- Assist in patient assessments and communicate care plans.
- Coordinate follow-up and referrals to clinical services.
- Complete program evaluation summaries and facilitate staff meetings.
- Assist with fiscal management of annual budgets.
- Provide outreach and education services to other professionals as appropriate.
Qualifications
- Bachelor’s degree in Social Work or related field.
- Minimum of 3 years of relevant experience.
- Knowledge of biology, clinical judgment, medicine, cultural awareness, and consulting.
- Skills in medical documentation, interpersonal communication, empathy, and decision-making.
