Life Coach, Tailored Day Services

Position Overview
We are seeking a dedicated Life Coach to provide individualized support to consumers, helping them identify barriers, engage in community activities, and access services as defined by their Person-Centered Plan and Individualized Personal Plan.

Key Responsibilities

• Individual Support: Provide personalized assistance in home or community settings, aligning with each consumer’s Individualized Personal Plan.
• Family Collaboration: Work closely with family members to identify needs and develop effective support plans.
• Community Integration: Develop opportunities for consumers to participate as valued members of the community, facilitating positive interactions and experiences.
• Activity Facilitation: Create and lead activities that build relationships, skills, and community engagement, including accompanying consumers to various locations.
• Advocacy: Act as an advocate on behalf of consumers, ensuring their needs and preferences are represented and respected.

Qualifications

• Education: Bachelor’s degree in a related field preferred.
• Experience: Experience working with individuals with developmental disabilities is advantageous.
• Skills:
  • Strong interpersonal and communication abilities.
  • Ability to develop and implement personalized support plans.
  • Proficiency in facilitating community-based activities.
• Requirements:
  • Valid driver’s license and reliable transportation.
  • Ability to travel with and/or transport clients throughout the local community.
