Health Coach (Consultant)

Position Overview
We are seeking a part-time Health Coach specializing in cardiovascular conditions and lifestyle risk factors. This consultant (W-9) role requires 20-25 hours per week, with potential for additional hours based on demand. You will support our members by providing personalized heart health coaching, utilizing behavior change science and evidence-based practices to promote cardiovascular wellness.

Key Responsibilities

• Member Coaching: Provide remote coaching via chat, video, phone, and email using AliveCor's platform.
• Personalized Support: Conduct individualized and group coaching sessions focused on cardiovascular lifestyle risk factors, such as stress reduction, nutrition, physical activity, and tobacco cessation.
• Progress Monitoring: Review member progress, offering empathetic feedback and guidance based on intake and ongoing data.
• Education and Resources: Guide members in preventing and managing cardiovascular diseases through education and resource sharing.
• Collaboration: Work with team members, including RNs, MDs, pharmacists, and product teams, to enhance the member experience and ensure continuity of care.
• Process Improvement: Identify areas for improvement in coaching processes to enhance quality and scalability.

Qualifications

• Experience: Proven experience in health coaching, particularly related to cardiovascular health.
• Skills:
  • Exceptional organizational and time management abilities.
  • Excellent written and verbal communication skills.
  • Empathetic and positive approach to coaching.
  • Ability to work independently and collaboratively in a dynamic environment.
• Passion: A strong commitment to improving heart health and empowering individuals through personalized coaching.
